At present, over 1,300 million people throughout the world are smokers, with 80 per cent living in low or middle income countries. It is estimated that, within 20 years, tobacco dependence could become the world's single largest cause of premature death or disability. However, despite wide dissemination of the Mayo Clinic recommendations (1999) on smoking cessation and treatment of tobacco dependence, few countries have taken sufficient action. This publication is based upon the WHO meeting, held in Moscow in June 2002, to establish a broad framework of policy recommendations which governments can progressively implement, taking into account different national capacities, infrastructure and resources, healthcare systems and culture. Issues discussed include: strategies for promoting a supportive environment for tackling tobacco dependence, selected country perspectives, behavioural and pharmacological interventions, capacity building and training of health professionals, the role of the WHO and its partners.
